Registration Form

Here are the simplified steps for filling out the registration form:

  • Fill out the registration form with your details using Option 1 (or) Option 2.
  • Submit the form.
  • We will review your submission.
  • You will receive course details, fees, and payment instructions.

Please choose one of the registration methods: either download and email the PDF form or just fill out the Google Form.

Option 1: M2Y Registration Form PDF

Option 1: Download and Email Registration Form

  1. Download the PDF registration form from the website.
  2. Fill out the form by hand.
  3. Take a photo or scan the completed form.
  4. Email the scanned form to admin@m2ysafety.com.
Option 2: M2Y Registration Google Form

Option 2: Fill Out the Google Form

  1. Open the Google Form link provided.
  2. Fill in the required details directly in the form.
  3. At the end, upload your signature (take a photo of your handwritten signature and upload it).
  4. Submit the form online.
